Falls Prevention

Focus: Reduce falls, falls-related injuries & death in people over age 65 in our communities

Indicator: Fall-related death rates

Measure: Age-adjusted death rate per 100,000 population due to falls.

Indicator: Fall-related emergency/hospital visits

Measure: Number of fall-related emergency department/ hospital visits per 100,000 older adults.

Indicator: Adults who have experienced a fall

Measure: Number of adults who have fallen at least once in the past 12 months.

Substance Misuse

Focus: prevent and treat substance misuse

Indicator: High School Alcohol use in the past 30 days

Measure: % of high school aged youth who report using alcohol in the past 30 days

Indicator: Adult Binge drinking

Measure: % of adults who report binging on alcohol in the past 30 days

Indicator: High School Binge drinking in the past 30 days

Measure: % of high school aged youth who report binging on alcohol in the past 30 days
